Perth Primary Care Network, in partnership with Curtin University of Technology, Perth North Eastern Medicare Local Ltd and Silverchain Nursing Association Inc has been selected to establish the Perth Central & East Metro Medicare Local.New key activities as a Medicare Local
To meet the strategic objectives for a Medicare Local, the Perth Central and East Metro Medicare Local will undertake a range of activities that will initially include the following.- Development and implementation of a regional after hours service plan in collaboration with existing service providers and local GPs to identify gaps in after hours services and expand the after hours workforce.
- Expansion of the nurse-led clinics to new sites to offer chronic disease management services.
- Development of an engagement model for Aboriginal people in health care, which can be replicated in different settings.
- Increasing the reach of Active Life-enhancing InterVEntion (ALIVE) suicide prevention services to additional sites, further linking these services to the Local Hospital Network.
Local partnerships
Perth Central and East Metro Medicare Local will be formed in partnership between the Perth Primary Care Network, Curtin University of Technology, Perth North Eastern Medicare Local Ltd and Silver Chain Nursing Association Incorporated.The partners forming the Perth Central and East Metro Medicare Local have established relationships with organisations such as Diabetes Western Australia, Multicultural Services Centre of Western Australia Inc., the National Asthma Council Australia, Swan City Youth Service Inc., the University of Western Australia and the Western Australian Practice Nurses Association.
